The National Wound Care Strategy Programme is working with the Royal College of Surgeons and the Vascular Society to develop national example referral forms to promote appropriate, timely referrals to vascular services for people with lower limb arterial disease and venous disease. We have developed two referral forms, one for peripheral arterial disease and chronic limb-threatening ischaemia and one for venous disease.
